Our HyCoSy Service (Hysterosalpingo Contrast Sonography) investigates for tubal occlusion, a significant cause of female infertility (- tubal factors account for approximately 20% of infertility cases). The fallopian tubes are not visible on routine ultrasound or x-ray and therefore require more specialised examination.
HyCoSy is a NICE approved non-invasive advanced ultrasound technique for the assessment of tubal patency. It can also detect for abnormalities of the uterus and endometrium.
As the HyCoSy procedure does not require the use of x-ray, there is no radiation dose and a comprehensive assessment of the uterus and ovaries is performed at a single visit. HyCoSy offers a number of advantages over HSG (Hysterosalpingography also known as uterosalpingography) which involves some radiation exposure as it involves an x-ray and in addition can only be performed from day 4 to day 10 of the menstrual cycle, whereas HyCoSy can be performed anytime up to day 22.
The process involves a catheter being inserted into the uterus and a 2D Transvaginal Ultrasound is performed to assess the uterus and ovaries. Saline solution is then instilled into the catheter to distend the endometrial cavity and a 3D Ultrasound of the uterus is obtained, and which is able to detect the presence of adhesions, intracavitary fibroids, polyps and congenital abnormalities. Following this, a specialised Ultrasound contrast is injected, which allows visualisation of the fallopian tubes and spill of contrast into the peritoneal cavity. As with HSG, antibiotic prophylaxis is given prior to the study as there is a very small risk of infection.
