Why is a Revision Hip Replacement Needed?

Hip replacements can wear out or fail for several reasons, including: 

  • Aseptic Loosening – The most common cause, occurring when the implant loosens from the surrounding bone without infection, leading to pain and reduced function. 
  • Infection – Bacteria can cause an infection around the implant, requiring its removal before a new implant can be placed. 
  • Fractures – A break in the bone around the implant, known as a periprosthetic fracture, may necessitate revision surgery. 
  • Implant Wear and Tear – Over time, the artificial joint may wear down, leading to pain, stiffness, and decreased mobility. 
  • Dislocation – If the hip implant becomes unstable and frequently dislocates, revision surgery may be necessary to correct the issue.

What to Expect from a Revision Hip Replacement

The Preoperative Assessment 

Before undergoing Revision Hip Replacement, a thorough evaluation is carried out to assess the condition of your hip and identify any underlying issues. 

This typically includes scans such as x-rays, CT scans, and/or MRI scans to assess implant positioning and bone quality to check for loosening or fractures around the implant. Blood tests may also be done to detect signs of infection or inflammation. If an infection is suspected, a joint aspiration may be ordered to analyse fluid from the hip joint. 

Once your consultant has reviewed the results, they will discuss the best surgical approach and the type of revision hip implant most suitable for your case. 

The Procedure 

Revision Hip Replacement surgery is more complex than a primary hip replacement, as it involves removing the existing implant, preparing the bone for the new prosthesis, and ensuring a stable reconstruction of the hip joint. The procedure typically involves: 

  1. Anaesthesia – The surgery is performed under general anaesthesia or spinal anaesthesia, depending on your medical condition and preference. 
  1. Incision – The surgeon makes an incision over the hip joint, often using the same incision as the original hip replacement, though it may need to be extended. 
  1. Removal of the old implant – Extracting the original hip implant can be challenging, especially if it is firmly attached to the bone or if bone loss has occurred. 
  1. Bone grafting (if required) – If there is significant bone loss, bone grafts or special implants may be used to restore stability. 
  1. Insertion of the new implant – A new hip implant is secured into place, ensuring proper alignment and function. 
  1. Closure – The incision is carefully closed, and a dressing is applied to the surgical site. 

The duration of Revision Hip Replacement surgery is usually longer than a primary hip replacement, often taking several hours due to its complexity.

Recovery and Aftercare

Recovery following a Revision Hip Replacement may take longer than a primary hip replacement due to the nature of the surgery.  

Your post-operative care plan will include pain management, with medication provided to help manage any discomfort in the days following surgery. A tailored physiotherapy programme will be developed to restore mobility and strength.  

Walking aids such as crutches or a frame may be required initially to support movement. Regular follow-up appointments with your consultant will be scheduled to monitor your progress and assess the healing process. 

Most patients can return to light activities within a few weeks, but full recovery and return to normal daily activities may take several months. Your consultant will provide personalised advice on when you can resume exercise, work, and other activities.
